RESUMO

Traditional Chinese medicine (TCM) has certain advantages in the treatment of chronic kidney disease-mineral and bone disorder (CKD-MBD). In recent years, there have been many studies on the treatment of CKD-MBD by Chinese medicinal compounds and monomers. As revealed by literature retrieval, the research on the mechanism of Chinese medicine in intervening in signaling pathways related to CKD-MBD was mainly based on self-made Chinese medicinal compounds, and the action pathways involved fibroblast growth factor 23/Klotho (FGF23/Klotho) signaling pathway, Wnt/β-catenin signaling pathway, receptor activator of nuclear factor-κB/receptor activator of nuclear factor-κB ligand/osteoprotegerin (RANK/RANKL/OPG) system, and other signaling pathways. TCM can improve calcium and phosphorus metabolism and bone metabolism disorder, and regulate inflammatory reaction, oxidative stress, apoptosis, and autophagy by regulating this series of signaling pathways for the treatment of CKD-MBD. This paper introduced the research results of these signaling pathways and the mechanism of TCM in the treatment of CKD-MBD in order to provide ideas and references for the related research of Chinese medicine in the treatment of CKD-MBD.

Objective:To evaluate the quality of the current foreign clinical practice guidelines (CPG) on obstetric venous thromboembolism (VTE), providing reference for standardizing the prevention and treatment of domestic VTE clinical practice.Methods:The GPGs for the management of obstetric VTE published abroad from inception to July 2020 were electronically searched. Two researchers independently evaluated the quality of the included guidelines in accordance with the internationally recognized guideline evaluation tool, AGREE Ⅱ.Results:A total of 12 guidelines, 2 recommendations and 1 consensus for obstetric VTE were included, covering 3 continents and 9 countries, and the published time span was 2009-2020. The standardized scores of the six domain (scope & purpose, stakeholder involvement, rigor of development, clarity of presentation, applicability and editorial independence) included in the guidelines were: 99.44%, 62.78%, 70.35%, 95.74%, 68.80% and 76.94% respectively, and the scores in each domain were ≥ 50.00%, indicating that the included CPGs were of good quality. Except for the domain scope & purpose, the ranges of the other five domains were ≥ 50.00%, suggesting that the quality of the CPGs differed significantly. Among the 15 included CPGs, 4 CPGs were level A (recommended), 11 CPGs were level B (recommended after being revised), and there was no CPG in level C (not recommended). The difference in the recommended content of obstetric VTE management mainly focused on the risk assessment, drug type, dosage and period.Conclusions:The overall quality of the CPGs for obstetric VTE management is high, while the quality of the CPGs varies greatly. In addition, there are differences in the recommended content of obstetric VTE management. More high-quality researches are required to provide evidence-based support for the improvement of the CPGs.

Antiangiogenic target therapy has been a hot topic in cancer treatment recently.Apatinib is a category 1.1 new medication developed domestically.It effectively inhibits angiogenic and exhibits promising anti-tumor activity in preclinical studies.Apatinib has been successfully applied in clinical trials of multiple malignancies,such as gastric cancer,lung cancer and breast cancer with satisfactory safety and efficacy profile.However,its mechanism of action is still not fully understood. Objective To explore the clinical value of CT for guiding interventional therapy of Budd-Chiari syndrome (BCS) with inferior vena cava (IVC) obstruction.Methods A total of 329 consecutive BCS patients with IVC obstruction underwent endovascular treatment.All the patients underwent CT examination prior to endovascular treatment,and the data were retrospectively reviewed.The accuracy,sensitivity,specificity,positive predictive value (PPV) and negative predictive value (NPV) of CT were compared with results of DSA.Results DSA showed partial obstructive IVC in 108 BCS patients and complete obstructive IVC in 221 patients.CT demonstrated partially obstructive IVC in 99 patients and completely obstructive IVC in 230 patients.CT revealed 15 false-negative and 6 false-positive results.CT diagnosis of puncture or not yielded an accuracy of 94.19%,sensitivity of 97.29%,specificity of 86.11%,PPV of 93.49% and NPV of 93.94 % for detection of BCS with partial IVC obstruction,while CT-based interventional treatment plan yield an accuracy of 97.55 %,sensitivity of 100 %,specificity of 92.52 %,PPV of 96.49 % and NPVof 100%.Conclusion CT can provide high diagnostic accuracy and sensitivity for BCS patients with obstructed IVC,therefore being helpful to planning interventional treatment of BCS.

Objective To investigate the diagnostic value of CT-guided craniocerebral puncture biopsy, and to discuss its clinical safety. Methods During the period from April 2013 to June 2014 at authors’ hospital CT-guided craniocerebral puncture biopsy was carried out in 23 patients. All patients had clinical symptoms or signs of nervous system. Imaging examination revealed that all patients had intracranial space-occupying lesions. CT-guided craniocerebral puncture biopsy was performed to make pathological or bacteriological examinations. The results were statistically analyzed. Results In this group of patients, definite pathological diagnosis was made in 19 cases, biopsy positive diagnosis rate of biopsy was 82.6% (19/23), among them oncology-pathological diagnosis was obtained in 14 cases (60.9%), mainly including glioma, non-Hodgkin lymphoma, dysembryoplastic neuroepithelial tumor, choroid plexus papillary tumor, epidermoid cyst, etc. Non-neoplastic lesion was confirmed in 5 cases (21.7%), including suppurated meningitis, cerebral gliosis hyperplasia, cerebral telangiectasis, etc. Small amount of subdural hemorrhage was observed in one case (4.3%). No severe complications, such as intracranial infection or epilepsy, occurred in all patients. Conclusion CT-guided craniocerebral puncture biopsy is minimally-invasive with fewer complications, it can provide definite diagnosis for intracranial lesions, therefore, this technique has a broad development prospect in clinical practice.

Objective To explore the effect of family mission intervention on family function and parental self-efficacy in children with malignant tumor. Methods 58 children with malignant tumor from 4 tertiary general hospitals in Tangshan city were included. Their parents were divided into control group (n=28) and intervention group (n=30). Both groups received routine treatment and nursing, while the inter-vention group received family mission intervention in addition. They were evaluated with Feetham Family Functioning Survey (FFFS) and the Self-Efficacy for Parenting Tasks Index-Toddler Scale (SEPTI) before and after intervention. Results After intervention, the total FFFS score (1.01±0.55), family and individual relationship score (0.79±0.64), and family and social relations score (1.21±0.92) were lower in the intervention group than in the control group (P<0.05). The scores of limit/constraint (4.19 ± 0.55), game (4.09 ± 0.32), and education (4.17 ± 0.78) of SEPTI were higher in the intervention group than in the control group (P<0.05). Conclusion Family mission intervention can im-prove the family function of children with malignant tumor, and improve the parents' self-efficacy.

Objective To investigate the influence of low-dose,long-term arsenite exposure on keratinocyte (HaCat) cell apoptosis and the expression of Bcl-2 and Bax proteins.Methods HaCat cells were exposed to different concentrations of NaAsO2[0(control group),0.05,0.10 μmol/L] for 15 weeks.A total of 10 000 cells in each group were measured to detect the level of apoptosis by flow cytometry.The protein expressions of Bcl-2 and Bax were determined by Western blotting method.Results The cell apoptosis rate was significantly different between groups (F =17.19,P < 0.05).Compared with that of the control [(1.42 ± 0.13)%],the apoptosis rates of the 0.05 and 0.10 μmol/L groups [(1.23 ± 0.08)% and (1.04 ± 0.06)%] decreased significantly (all P < 0.05); the 0.10 μmol/L group was significantly lower than that of the 0.05 μmol/L group(P < 0.05).The protein expressions of Bcl-2 and Bax were significantly different between groups (F=107.38,346.45,all P< 0.05).The protein expression of Bcl-2 of the 0.05 and 0.10 μmol/L groups were (143.89 ± 7.74)% and (199.96 ± 12.18)%,respectively,which were significantly higher than that of the control group[(100.00 ± 1.45)%,all P < 0.05] ; the 0.10 μmol/L group was significantly higher than that of the 0.05 μmol/L group (P < 0.05).The protein expression of Bax of the 0.05 and 0.10 μmol/L groups were (70.78 ± 1.53)% and (54.80 ± 1.34)%,respectively,which were markedly lower than that of the control group[(100.00 ± 3.09)%,all P < 0.05]; the 0.10 μmoL/L group was significantly lower than that of the 0.05 μmol/L group(P < 0.05).Conclusion Decreased level of apoptosis induced by low level and long-term arsenic exposure may be associated with increased expression of Bcl-2 protein and decreased expression of Bax protein.

To find a new method to identify Mori Cortex and its adulterants by analysis their ITS2 sequence of barcode,total genomic DNA was isolated from Mori Cortex and its adulterants. Nuclear DNA ITS2 sequences were amplified,and purified PCR products were sequenced. Sequence assembly and consensus sequence generation were performed using the CodonCode Aligner V3.0. The Kimura 2-Parameter (K2P) distances were calculated us-ing software MEGA 4.0. Identification analyses were performed using BLAST1, Nearest Distance and neighbor-joining (NJ) methods. The results showed the intra-specific genetic distance of Mori Cortex was 0, which were lower than inter-specific genetic distances between Mori Cortex and its closely related species (0.003-0.343). The ITS2 region is an efficient barcode for identification of Mori Cortex and its closely related species, which provides a scientific basis for fast and accurate identification and new method of Mori Cortex.

RESUMO

Objective To discuss the operational technique and clinical effect of metallic stent placement in treating malignant gastroduodenal obstruction.Methods Metallic stent placement under fluoroscopic monitoring and endoscopic direct vision was performed in 47 patients with malignant gastroduodenal obstruction.A total of 54 metallic stents was used.Results The procedure was successfully completed in all 47 patients.During the follow-up period,all the patients could take liquid or ordinary diet and were markedly relived of vomiting.The living quality was much improved and no serious complications occurred.Conclusion Under fluoroscopic monitoring and endoscopic direct vision,stent placement is a safe,effective,technically-simple and time-saving procedure for the treatment of malignant gastroduodenal obstruction with less sufferings to the patient.Therefore,it is definitely worth popularizing this technique in clinical practice.

Objective To summarize the clinical manifestations and traditional Chinese syndromes of 27 patients with confirmed swine flu(H1N1 flu),and to investigate the therapeutic effect of therapy of combining cooling method with warming method for swine flu.Methods Twenty-seven patients with confirmed swine flu were enrolledinto the study.The swine flu patients were treated with Yinqiao Chaigui Decoction and Baihu Qingjie Decoction on the basis of staging diagnosis and treatment.The clinical manifestations and traditional Chinese medical syndromes of the patients were observed,and the therapeutic effect was evaluated.Results(1) Clinical manifestations: in 27 swine flu patients,fever accounted for 100%,aversion to cold accounted for 85.2%(23 cases),cough accounted for 92.6%(25 cases),expectoration accounted for 81.5%(22 cases),sore throat accounted for 77.8%(21 cases),and general soreness accounted for 74.1%(20 cases).(2) Therapeutic effect: 24(88.9%) patients were markedly effective,3(11.1%) effective,and the total effective rate was up to 100%.(3) Blood analysis: except for the white blood cell count,the differences of the other blood analysis parameters were significant(P

RESUMO

The present study was undertaken to identify some afferent connections of the ventral and dorsal parts of pontine tegmentum in guinea pigs by using horseradish peroxidase retrograde microelectrophoretic technique.After introducing HRP into the ventral part of pontine tegmentum, numerous labeled cells were observed in the mesencephalic central gray and related reticular formation, which is equivalent to the MSR (Mesencephalic Stepping Region) ventral to the surface of the superior colliculus. Labeled cells could also be identified at the following sites: nucleus cuneiformis ventral to the surface of inferior colliculus, nucleus tractus spinalis nervi trigemini, nucleus reticularis gigantocellularis, nucleus medialis vestibulares, nucleus lateralis vestibulares, nucleus loci coerulei and its ventral part, gray matter of the cervical enlargement of spinal cord.Following injection of HRP into the dorsal part of pontine tegmentum, substantial amount of labeled cells were observed in the nucleus reticularis pontis caudalis and nucleus reticularis gigantocellularis Labeled cells gould also be identi- fied in the nucleus medialis vestibulares, nucleus lateralis vestibulares and nucleus cuneatus. Only a few HRP cells appeared around the nucleus tuber in the midbrain and no labeled cells have been seen in the nucleus loci coerulei, as well as the gray matter of cervical spinal cord.
